Job descriptionLifeLong Medical Care has an opportunity for a General Internist or Family Practice Physician to join our health centers providing primary care to a diverse patient population. The ideal candidate would be taking on a well established patient panel with a focus on Diabetes management and Hypertension. Must have a commitment to providing high-quality outpatient primary care dedicated to building a collaborative team model of care, and ideally experienced working with the under-served.
